9. Parental Consent & Declaration


By registering your child as a member of the Club and/or allowing them to take part in Club activities:

  1. You confirm that your son/ daughter is physically fit and capable of full participation.
  2. You agree to notify the Club of any changes to the medical information you have provided.
  3. You give you permission for team / managers appointed by the Club to obtain emergency medical treatment for your son / daughter where they consider this necessary.
  4. You consent to your child travelling to venues for matches and training by transport provided by the Club which may include travelling in other players' private cars.
  5. As part of the Club’s adoption of England Hockey’s Safeguarding and Protecting Young People in Hockey Policy, the Club observes England Hockey’s Guidance for Taking and Using Photographic and Recorded Images of Young People. However, the reality of modern life means it is practically impossible for the Club to police absolutely whether spectators or third parties are takings photos or videos of Club activities (including matches). Whilst this is particularly true when those activities take place in public or semi-public venues, this can apply even to activities at the Club’s own facilities. By allowing your child to take part in Club activities, you accept this situation.
